A price that looks half as good as a competitor's is often simply quoted the other way round. Always ask, and always get it in writing. ## 2. What actually drives the price Four things move the number, and only two of them are about sound. Factor What it means for the price **Technology level** The biggest single driver. The same physical hearing aid is sold at three or four levels — premium, advanced, standard, essential — with features progressively switched off. On one current flagship platform, the premium level offers 48 processing channels and three advanced focus streams; the advanced level offers 36 channels and one focus stream; and the headline conversation feature is reduced further at the standard level and absent altogether from the smallest custom model [6]. Under Indian GST, **complete hearing aids falling under HSN heading 9021 attract a nil rate** - that is, zero per cent [3][4]. There is a genuine complication, and we should report it accurately rather than oversimplify. In a case involving Sivantos India, the Appellate Authority for Advance Ruling held that **parts and accessories of hearing aids falling under tariff item 9021 90 10 are not covered by the exemption and are taxable at 18 per cent** [3][4]. So the device itself is nil rated, while certain separately supplied parts and accessories may attract tax. **What to do with this information** Ask for a proper tax invoice that shows the HSN code and the tax treatment line by line. If GST is being charged on the hearing aid itself, ask the seller to explain the classification they are using. Please treat this as information to raise with your seller and, if a large sum is involved, with your own accountant — not as tax advice.* ## 4. The ₹20,000 question: hearing aid or ear machine? A great deal of what is sold cheaply online in India, often under the name *ear machine*, is a personal sound amplifier rather than a hearing aid. The distinction is not snobbery. It is regulatory and it is clinical. To practise clinically in India an audiologist must be registered with the Rehabilitation Council of India Nobody. It arrives in a box **Adjustable later** Yes, repeatedly, as your hearing changes No, or only crudely **A point about safety, not sales** An amplifier with no properly controlled output limit can deliver sound at levels that are uncomfortable and, in principle, harmful - particularly to an ear that already has damage. That is the real reason the output ceiling is set by a professional against your audiogram. **There is also an important legal gap that Indian buyers should know: while an audiologist must be RCI-registered to practise clinically, there is no equivalent restriction on who may sell a hearing aid. The qualification of the person fitting you is therefore something you have to check yourself.** ## 5. Now the part nobody wants to hear: why the discount is the wrong thing to optimise Here is the behaviour we see almost every week. A patient visits three or four places, collects quotations for the same model, and buys from whoever knocks the most off. It feels like the rational thing to do. With a mobile phone or a television it would be. A hearing aid is not that kind of purchase, and the reason is simple: **the device you buy is not the product. The product is the device plus the programming plus several years of adjustment.** The hardware is only the part you can see. ### What the independent evidence actually shows We would not expect you to accept that from a clinic that benefits from saying it. So here is measured, third-party evidence. The only variable was whether a professional measured the output inside the ear canal and corrected it. **If you buy a premium hearing aid at a 30 per cent discount and it is never verified on your ears, you may be wearing a device performing well below what you paid for - and you will probably never know, because under-amplification does not sound wrong. It just sounds quiet, and people assume that is what their hearing loss sounds like now.** ### The uncomfortable arithmetic Consider two patients, both spending about ₹2,00,000. This is an illustration, not a case study - but the mechanism it describes is real and is supported by the evidence above. Patient A — bought on discount Patient B — bought on service **What they paid** ₹2,00,000 for a premium device, heavily discounted ₹2,00,000 for a mid-range device with a full service package **Fitting** Manufacturer default settings, no verification Real-ear measurement, output verified in the ear canal **Follow-up** Sold and delivered. What "service" actually means over five years When we say a hearing aid needs service, patients often picture an occasional cleaning. It is considerably more than that, and it is worth listing so that you can ask what is and is not included in any quotation. Over the life of the device Why it is needed **Real-ear verification at fitting** To confirm that the sound reaching your eardrum matches the prescription target for your loss. The evidence above shows this is where most of the performance lives **Fine-tuning appointments in the first months** Your brain adapts to amplification gradually. The setting that is right on day one is rarely right on day sixty **Reprogramming as your hearing changes** Hearing loss is usually progressive. A device programmed to a four-year-old audiogram is programmed to the wrong ears **Annual hearing re-assessment** To catch that change, and to catch medical problems early **Wax filter and dome replacement** Typically every one to three months. Wax blockage is one of the commonest causes of a hearing aid that has apparently stopped working **Tube and receiver replacement** Receivers fail, especially in humid climates. Replacement is routine, not exceptional **Deep cleaning and dehumidifying** Delhi summers, monsoon humidity and perspiration are among the commonest reasons devices come back for repair **Firmware and app updates** Manufacturers issue meaningful updates. The five-year cost of ownership Hearing aids are normally expected to last around three to five years, sometimes longer with good care. That is the right window over which to judge a price. A provider confident in their service will do it without hesitation. ## 8. What CGHS and insurance actually cover - **CGHS.** For beneficiaries of the Central Government Health Scheme, the reimbursement ceiling is **₹30,000 per ear, inclusive of GST and all applicable taxes**, and up to ₹60,000 for a clinically justified bilateral fitting. Replacement is permitted only after five years, and the device supplied must carry a minimum three-year comprehensive warranty [9]. - **What that means in practice.** On a premium device, CGHS covers a fraction of the cost and the balance is payable by the patient. Anyone buying at the top of the market under CGHS should do that arithmetic before committing, not afterwards. - **Private health insurance.** Hearing aids are commonly excluded or heavily sub-limited in Indian health insurance policies. Check the specific policy wording rather than assuming, and get any confirmation in writing from the insurer rather than from a seller. - **EMI.** Widely offered in India. Compare the total amount repayable, not the monthly figure, and confirm whether the interest is genuinely zero or built into the price. - **Disability certification.** Where hearing loss meets the statutory threshold, a disability certificate may open access to government schemes and concessions. Ask your ENT or audiologist whether assessment is appropriate in your case. ## 9. How to compare two quotations properly Print this. Take it to every clinic you visit. Two quotations are only comparable if every line below is filled in for both. Red flags worth walking away from Red flag Why it matters **A large discount offered on the spot, valid today only** Pressure selling is not a pricing strategy. Ask for it **No real-ear verification, and no clear explanation of why** The evidence above shows this is where the performance lives **Refusal to put the quotation in writing** Everything you were promised verbally will be disputed later **The fitter's qualification is vague** Ask directly whether they are an RCI-registered audiologist **GST charged on the hearing aid with no HSN code shown** Complete hearing aids under HSN 9021 are nil rated. Ask for the classification **A brand you cannot find any independent information about** If no independent laboratory or trade publication has ever assessed it, you are relying entirely on the seller **Pressure to buy both ears when you asked about one, with no clinical reason given** Bilateral fitting is often clinically correct - but it should be explained, not assumed **No trial period** You cannot judge a hearing aid in a quiet consulting room ## 11. The bottom line Hearing aid prices in India are genuinely confusing, and some of that confusion is convenient for sellers. The price bands in this guide should at least tell you whether a quotation is in a sensible range, and the comparison checklist should let you judge two offers on the same terms. But the more useful conclusion is the less comfortable one. A hearing aid is not a gadget. It is a medical device that has to be matched to your particular ears and then maintained, adjusted and reprogrammed for as long as you wear it. The independent evidence on this is now unusually clear: **the same device can perform below the generation it replaced or at the top of its class, depending entirely on whether somebody did that work properly**. So by all means negotiate. Ask for a fair price, compare properly, and do not overpay. But if the only question you ask is who will give the biggest discount, you may win that negotiation and still end up with a device in a drawer.
